Select the answer that most accurately describes your organization today.

1. Is multifactor authentication required for email and important business accounts?

This is the additional verification code used after entering a password.

2. Are company computers protected by centrally managed security software?

3. Are security updates installed promptly on computers, servers, and network equipment?

4. Are administrator privileges limited to authorized personnel and separate from everyday user accounts?

Employees should not routinely work from accounts that can install software or change security settings.

5. Are important business files backed up automatically?

6. Is at least one backup protected from ransomware or accidental deletion?

For example, an offline, immutable, or separately secured backup.

7. Are Microsoft 365, Google Workspace, or other critical cloud services backed up separately?

Cloud availability does not always include complete protection from deletion, account compromise, or retention limits.

8. Has your organization successfully tested restoring files and critical systems within the past year?

9. Does your organization have a written plan for operating during a major technology outage?

10. Does your organization have a written cybersecurity incident-response plan?

The plan should explain who to contact and what to do after ransomware, account compromise, data exposure, or another suspected incident.

11. Are internet, power, and critical-system failures addressed with backup options or documented procedures?

12. Are key technology vendors, contacts, system dependencies, recovery priorities, and emergency steps documented and reviewed?

13. Do employees receive regular cybersecurity and phishing-awareness training?

14. Are former employees' accounts and access removed promptly using a documented offboarding process?

15. Does each employee use an individual account instead of shared usernames and passwords?

16. Are business email, mobile devices, and remote access protected and monitored for suspicious activity?

Examples include phishing protection, device encryption, screen locks, secure remote access, and unusual-login alerts.

17. Does your organization maintain an accurate list of computers, software, cloud services, warranties, and licenses?

18. Are IT issues, recurring problems, and service requests tracked in one organized system?

19. Has your organization completed a professional technology or cybersecurity risk review within the past 12 months?

20. Are applicable compliance obligations and cyber-insurance requirements identified, documented, and reviewed?

Examples may include HIPAA, PCI DSS, CMMC, contractual security requirements, or cyber-insurance controls.
